Read below for more advice about completing online forms.Applications: DSACF Scholarship Application (PDF): This form is the application students should use to apply for most Duluth Superior Area Community Foundation scholarships. This is the form sometime refered to as the Common Application. Minnesota Power Community Involvement Scholarship: Students should visit the Minnesota Power Web site to learn more and download an application for the Community Involvement Scholarship. The application for the 2010-2011 academic year will be available in early September of 2010. Minnesota Power New Generation Scholarship: Students should visit the Minnesota Power Web site to learn more and download an application for the New Generation Scholarship. Greg Irons Student Application (PDF): Students should use this form to apply for the Greg Iron Student Scholarship. Greg Irons Staff Nomination Form (PDF): People interested in nominating an employee (faculty or staff) of the Duluth Public Schools for the Greg Irons Staff Award should use this form.
DSACF Scholarship Recommendation Form (PDF): Teachers, counselors, advisors and administrators should use this form to recommend a student for a particular scholarship.
DSACF Transcript Release (PDF):
This form can used for Duluth students to request a transcript from
their school be sent to the Duluth Superior Area Community Foundation
for academic review. Renewal Form (PDF): Students who would like to renew a multiple year scholarship should use this form to apply for a continuation of their award. Scholarship Final Followup Form (PDF): Students who are graduating and otherwise finishing their scholarship should use this form to report back to the Community Foundation about how they made use of their award.
The above PDF forms are designed to be completed and saved with Adobe Reader. Most of them can be opened with web browsers (Internet Explorer, Firefox, (or for Macs, Preview) etc), HOWEVER, by doing this, the save function may not work correctly. It is therefore important to use Reader to create a document which is fill-able, save-able and return-able. The link to download the newest version of Adobe Reader is here. Once you have the correct program, save as/download the form to your desktop; open the Reader application and then open the document with the 'Open' command under the 'File' dropdown. After you have completed the form, 'save as' to your desktop or other folder, create an email and attach the saved PDF. Even more thorough instructions are available on the FAQs and Tips page.
